    Reading the report amongst everything else … I was genuinely shocked to read that both planes had their ejection seats deactivated! Probably would have saved their lives tbh! Would love to know the reasoning behind that decision (cost?).

      I think it is pretty common practice for jets operated privately, although I’m not sure why. Perhaps an on the ground safety thing as maintaining an ejection seat is very difficult

Instead a crumb trail of blame factors to promote the judging of deceased in the kangaroo court of public opinion. Keep in mind that the ATSB are the investigators, and will make findings, but it is the role of CASA to regulate aviation safety.

    In my basic flying training I was introduced to imc. Wore a shroud to limit view to the instruments only, instructor monitoring. Now qualifies SEP land,[16 yrs], I avoid flying if a risk, and even if a sea fog rolls in gain height above use vor and radio for vectors, life is precious.

    What a load of bs. It was nothing to do with briefing. The pilot of the lower jet just stuffed up. Rule number 1 keep your eye on the other aircraft and dont hit it. Thats all that went on here. Poor pilot execution of a straightforward manoeuvre (well straightforward for an experienced formation rated pilot..

      My thinking is there could have been a number of factors, such as updrafts, pitch up to compensate for reduction in speed, and high pilot mental load due to ineffective CRM. No factor would have been a problem if they had flown as briefed, which I would guess is why ATSB didn't condemn the maneuver itself but the lack of planning for it.

    Did the ATSB miss yet another glaring CASA failure? The report states (I have read the full report) that, the pilot of Viper 1 had the, "required ratings, endorsements and type training, to operate the S211, as a pilot in command". However, at the same time he had a safety pilot medical restriction, that any aircraft flown by the pilot, be configured in a side-by-side, seating configuration. How can he, at the same time, be allowed to fly an aircraft with tandem seating as a "pilot in command"? As the regulating authority, shouldn't CASA, at the same time as issuing his safety pilot safety medical restriction, have revoked his rating to fly the S211 as a "pilot in command"?

      Maintaining a big list of each individual airplane a given individual can or can't fly would be a big, complex, time-consuming, and completely unnecessary task. So ratings are given out for groups of aircraft, and if an an aircraft belongs to more than one group, you need _all_ the associated ratings/endorsements/type ratings to legally fly that aircraft. In other words, there's no such rating as "fly the S211 as a 'pilot in command'" for CASA _to_ revoke. There is just a "single-engine airplane" rating, which he did have. However, the S-211 _also_ requires an appropriate medical certificate, which he didn't have, so it was still illegal for him to fly it.

    So the better part of a year later, a bunch of armchair generals concluded that sometimes, things don’t go the way we want. We hear ‘pilot error’ and we equate it with gross negligence. Incorrect. Pilot error happens all the time. Amazingly frequent. Sometimes it doesn’t end well. Usually, it’s fine. These weren’t your Dad’s 152s. This wasn’t a QantasLink flight. Subsonic, yes, but still fast jets. In close formation flying. Blue angels. Thunderbirds. Black knights. Red Sales…etc…even the very best pilots are still human. some of the families will know exactly what I’m referring to. Some might not. However, the very reason there were cameras in these ships is because this wasn’t routine flying. And because we’re not talking air transport, if there wasn’t any risk, then what the bloody hell is the point…

      Anyone can fly, assuming they have the correct training. While training in a jet seems over the top, if they started with basics and worked through a standard syllabus anyone can fly. RAAF are starting training in Turbine PC-21's, which to me who started in gliders and then cessnas, is an advanced aircraft!

Why are we still seeing GA midair’s? Shit, we’re still seeing Dash-8s nearly colliding with pipers at regional airports because of radio procedures. The answer? Flying. Like driving. Like bicycling. Like walking. Has inherent risks. And shit, if it was routine, and risk free, there wouldn’t have been cameras there in the first place.

    Human factors would seem a significant issue. The mishap pilot owned the aircraft and the operation (which was created specifically for the documentary being filmed) and though by no means inexperienced, was relatively inexperienced. An intelligent gentleman, who nonetheless carried out an unbriefed maneuver without warning during a close formation flight. That is not disciplined flying. Who in the operation was able to impose discipline on the owner-operator-pilot? The relatively inexperienced owner-pilot would likely have had, and felt, a level of authority disproportionate to his experience, whilst the pilots with much more experience, not only in hours, but also in quality may have been more tolerant of mis-steps in consequence of the ownership status of the gentleman. In no way do I disparage the mishap pilot, but I can't help but speculate that the way that he painted himself into this corner was by a normalisation of deviance eroding the margins between himself and disaster. Nor do I criticise the other pilots for going along with an impromptu change after the initial unbriefed action. No doubt the ATSB will in due course provide further lessons learned from this horrible event, though not worth the price paid. Flying is a dangerous business made safe by deeply considered risk management - unwarned, unbriefed and impromptu actions in formation flight make it difficult to successfully manage risks. The human reality is that we are all susceptible to these kinds of errors, so we have to work hard to minimise them.

    Presumably the pilot had a commercial licence and thus a reasonable number of hours logged?

    I have to say. After looking at this plane I have like my race cars and top tier open wheel racing teams I’m not impressed. The front of my RV6A has 2.2* of negative caster. This is so backwards it ridiculous. I can’t believe a company can’t even do that. Let alone make it fly. I’m actually pisses these morons make planes. Complete morons.
